How to Order a Sizing Kit

Tell me about your dog by completing the below form.

Do not--I repeat, do not send any money at this time.

Upon receipt of the below form, I will either contact you for additional information, or I will provide you with a detailed description of your kit.  I want you to see what you're buying before you send any money.  Included in my e-mail will be specific instructions for ordering your kit.

Comments -- Include anything that you think is helpful.  For example--My dog has an unusually small muzzle.

 

Special Needs -- For example, I was once asked to make articles for a dog that had no front teeth.
